The character sheets go as follow:
DN Username: (your Dueling Network username)
Character Name: (name yourself for the RP)
Description: (describe yourself; optional)
Alignment: (Hero/Villain)
Starting Deck: (Name of deck your first owned in RP i.e. 1 of the 6 atributes)
Once you have posted your character sheet, your profile will be updated and you can start playing the RPG just fine.
